One thing to watch for when you're adding a medallion is that the adhesive you're supposed to use can actually pull off the first few layers of paint. My contractor simply affixed it to the ceiling when he installed my domed deco chandelier, which held it in place. Only one tiny part wasn't flush with the ceiling, so we added silicon there in a small dab.
